WAIVER OF MEDICAL COVERAGE DISCLOSURE

As required by the Patient Protection and Affordable Care Act (ACA), full-time employees are given the opportunity to enroll in medical health coverage offered by the Byron Union School District. Employees have the right to decline, or waive coverage. If employees waive coverage for themselves and/or dependents under the Byron Union School District's medical health plan, employees are required to show proof of other employer, non-individual market health plan coverage* by providing proof of coverage or by submitting a completed Waiver Of Offer of Medical Coverage Form annually during Open Enrollment.

The following may apply if you decide to waive coverage:

• If you waive this coverage and do not obtain coverage on your own, you could be subject to a penalty under the individual responsibility requirement of the ACA and will not qualify for government subsidies to purchase individual health insurance on the Marketplace.

• If you waive coverage, you will only be allowed to enroll in the Byron Union School District medical health plan if you have a qualifying event (e.g. marriage, divorce, birth/adoption, loss of coverage, change in job status). You must request to enroll in the plan within 60 days of the qualifying event.

HOW TO ENROLL IN THE CASH-IN-LIEU OPTION:
Employees who wish to waive the benefit options offered by the district must provide proof of alternative coverage by one of the following options:

  • Upload a completed Medical Waiver Form completed and signed by the employer/agency providing the alternative medical coverage to the Benefits Bridge Portal. Electronic or “wet signature” is required.
AND (if you are unable to provide a signature from the employer/agency providing alternative coverage)

  • Upload a copy of the Benefit Coverage Summary to the Benefits Enrollment Forms Portal that includes all of the following information*:

    • Name(s) of individuals covered under the policy

    • Type of coverage (Must include medical)

    • Start and end dates of coverage (must cover from 1/1/2024-12/31/2024)

* Please note that a copy of a medical insurance card will only be accepted if all of the information above is included on the card.

Eligibility for the "cash-in-lieu” option is reestablished every year during Open Enrollment.  This includes completing a new Waiver of Group Medical Coverage and providing the relevant proof as needed.

Participants in the "cash-in-lieu” must notify BUSD Human Resources immediately if they have a loss of coverage.  Human Resources will work with the employee to enroll them in a District medical plan.
